The following provides some important information about insurance,
appointments, and financial arrangements for dental care.

Dental Treatment and Financial Responsibility

A treatment plan will be prepared for you detailing your needed dental care as well as related costs. Orange Tree Dental is a fee for service dental office payment or insurance co-payments are due when services are rendered. We are sensitive to the fact that some patients may need alternative payment options, and therefore accept any of the following:

1. Mastercard, Visa, American Express, Discover
2. Care Credit Monthly Payment Plan (This is a separate line of credit with no annual fees and does not affect the balance of other credit cards),
3. Automatic Payment Plan (Equal monthly payments through check debit)
4. Cash or check Payment plans our arranged for services that exceed $300.00

Appointments
Appointment times are reserved especially for you. If you must change your appointment time, we ask that you advise us immediately. When given a 24 hour notice you will not be charged a $25 room set up fee.

Regarding Dental Insurance
I f you have dental insurance, we will help you optimize your insurance benefits. We will ask that you pay any deductible, estimated patient share or non-covered services at the time of treatment by one of the payment options mentioned Please Be aware that Insurance coverage is estimated and that your actual indemnity may Be more and that you are responsible for whatever portion is not covered by your Insurance. Balances remaining over 45 days will be subject to a I5 % monthly finance charge.
